Forms & Downloads

KYC, SIP, redemption, nomination and change of details. Tell us what you want to do, and we send you the latest form and help you fill it in.

Which form do you need?

Most people need only three: a KYC form once, a transaction slip or SIP form to invest, and a nomination form so the family is protected.

Everything else is for changes later on. Many requests can now be done online without paper. Before you print anything, send us a message: we will tell you the quickest route for your request, send the current version of the form, and check it before it goes in, so it is not rejected for a missing signature or a wrong box.

Commission (brokerage) disclosure

What Finpotters earns from each fund house, shown openly before you invest.

When you invest in the Regular Plan of a mutual fund through us, the fund house pays us a commission out of the scheme's expenses. The rates differ by fund house and by scheme. Our latest commission rate sheets are kept in one folder, so you can see them at any time.

Rates change from time to time. For the current rate on any scheme, just ask us.

Ask us for any form on WhatsApp

Fund houses and registrars change their forms often, and an old version can be rejected. So we send you the current one, for your fund house, marked where to sign.

1. Tell us the task"Change my bank account", "add a nominee", "start a SIP".
2. Get the right formOr a link, if it can be done online.
3. Fill and signFill it in as the form asks. Every holder signs as in the folio records.
4. We check itBefore it goes in, and we tell you when it is processed.

The common forms, and what each is for

ForFormWhen you need it

Changing a SIPSIP pause or cancellation formTo pause (where the fund house offers a pause) or stop a SIP. The units you already hold stay invested.
Taking money outRedemption request; switch, STP and SWP formTo sell units, move money between schemes, or set up regular transfers (STP) or withdrawals (SWP).
Family protectionNomination form; transmission formTo add or change nominees, and for a nominee or legal heir to receive units after a death.
Your detailsChange of bank details; change of address, mobile number or emailWhen any of your details change. Often possible online.
TaxForm 121 (it replaced Forms 15G and 15H from 1 April 2026, under the Income-tax Act, 2025)If you are a resident individual or HUF and the tax on your estimated total income for Tax Year 2026-27 is nil, this declaration asks the payer not to deduct tax at source, for example on mutual fund dividends. NRIs and companies cannot use it.

Scheme documents: SID, SAI and KIM

Before you invest in any mutual fund scheme, you should read its offer documents. There are three.

SIDScheme Information Document: the scheme's objective, where it invests, its risks and its costs.
SAIStatement of Additional Information: facts about the fund house, its sponsors and its rules.
KIMKey Information Memorandum: a short summary of the SID that must accompany every physical application form.

These documents are published by each fund house on its own website and are updated often. For any scheme we discuss with you, we send you the latest SID, SAI and KIM, or the direct link to them.

Common questions

KYC, nominations, online changes and forms from abroad.

Why send forms on request instead of keeping PDFs here?
Fund houses and registrars update their forms often, and an old version can be rejected. When you ask, we send the current version for your fund house, and tell you where to sign and what to attach.
Do I need a new KYC for every fund house?
No. Once your KYC is validated by a KYC Registration Agency, other fund houses can use the same record, so you do not repeat it. If your details change, such as your address or mobile number, you update it once.
Is nomination compulsory?
For a folio in one name opened on or after 1 September 2026, yes in effect: under SEBI rules you must either add a nominee (up to three) or formally opt out. For older folios and joint folios, nomination is optional. Adding a nominee is strongly recommended: it makes the transfer of units to your family much simpler.
Can I do these changes online?
Many can, including SIP changes, redemptions and some updates to contact details. We will tell you whether your request can be done online before you fill in any paper.
I live abroad. Can I still use these forms?
Yes. NRIs often need extra documents, such as overseas address proof and a copy of the passport. See our NRI Guide or ask us for a checklist for your country.
Please note: Mutual Fund investments are subject to market risks, read all scheme related documents carefully.
